Intake Appointments Include:

An in depth review of your intake paperwork & discussion regarding the findings from your intake assessment and questionnaires.
Individualized treatment plan tailored to your needs including objective goals, as well as an estimation on how long therapy may last based on your presenting concerns.

Insurance Information

I currently accept Blue Cross Blue Shield, Aetna & Cigna insurance. Private Pay clients can pay for sessions with cash, check, or credit card with payment being taken through a secured platform.

For any insurance not listed above I am considered an Out-of-Network Provider.

I may be able to still work with you if you have an insurance I don’t currently accept with the option of submitting a superbill to your insurance to seek partial reimbursement. If you would like a superbill, one will be provided for you after each session. To Make Superbills Easier For You…
Zoe Mental Wholeness Clinic has partnered with the Reimbursify app for those who wish to pay out of pocket and seek out-of-network reimbursement. Once you sign up with their app and send Reimbursify a photo of your insurance card, you can upload the superbills I provide to you to simplify the reimbursement process from your insurance. Reimbursify will submit your superbills to your health insurance on your behalf. Reimbursify is safe, secure, and HIPAA-compliant. Current client’s see link below for easy submission of superbills.

Does my health insurance plan include mental health benefits?
What is my deductible? 
How much of my deductible has already been met? 
What is my coinsurance? 
Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
Do I need written approval from my primary care physician in order for services to be covered?

What do you require for me to get reimbursement for outpatient mental health services using a superbill? When and how do I submit these claims? What is the estimated time period from submission to reimbursement?
